In our capacity as a “Data Controller” and pursuant to Article 10 of the Law on the Protection of Personal Data No. 6698 (“KVKK”), we wish to inform and enlighten you regarding our personal data processing activities.

Purposes of Processing Personal Data Your personal data is processed by the Company for the following purposes:

• Fulfilling obligations set forth in relevant legislation, primarily the Basic Law on Health Services No. 3359, the Regulation on Private Health Institutions Providing Oral and Dental Health Services, and the Regulation on the Processing and Privacy of Personal Health Data.
• Protection of public health, execution of medical treatment and diagnostic services, and the development of related services and methods.
• Creating and tracking visitor records, and conducting storage and archival activities.
• Conducting advertising / campaign / promotion processes.
• Conducting finance, accounting, and contract affairs.
• Conducting post-service support services.
• Conducting customer relationship management processes and carrying out activities for customer satisfaction.
• Confirming relationships with contracted institutions.
• Ensuring physical space security and the protection of patient information.
• Within the scope of the Law on the Regulation of Electronic Commerce No. 6563, communications are sent using your contact information only if your explicit consent is obtained. You may opt-out of the messages you have approved at any time.

Method and Legal Basis for Collecting Personal Data The method of collecting your personal data may vary depending on your relationship with the Company but may be gathered through automated or non-automated means, via the website, call center, or by the data subject’s verbal, written, or electronic declaration at our hospital and branches. Your personal data is collected lawfully in accordance with the conditions stipulated in Articles 5 and 6 of the KVKK, primarily based on legal grounds such as being prescribed by law, being necessary for the performance of a contract, fulfilling the legitimate interest of the data controller, or obtaining explicit consent.

Transfer of Your Personal Data As the Company, due to the importance we place on the privacy of your personal data, we only share it with legally authorized institutions and organizations, except in situations required by our legal relationship with you.

Retention Periods for Your Personal Data Your personal data is stored for the period specified in the relevant legislation, if such a period is defined. If no retention period is stipulated in the legislation, your data is stored for the duration required by our relationship and as specified in the contract concluded with you.

Your Rights Under the KVKK As the data subject, you have the following rights regarding your processed personal data:

• Learning whether your personal data is processed,
• Requesting information if your personal data has been processed,
• Learning the purpose of processing personal data and whether they are used in accordance with their purpose,
• Knowing the third parties to whom your personal data is transferred, domestically or abroad,
• Requesting correction of your personal data if it is incomplete or incorrectly processed,
• Requesting the deletion or destruction of your personal data within the framework of the conditions stipulated in the KVKK legislation,
• When you request the correction of incomplete or incorrect data or the deletion or destruction of your personal data, requesting that this situation be notified to the third parties to whom we transferred your personal data,
• Requesting compensation for damages if you suffer damages due to the unlawful processing of personal data.

You may apply to the patient communication unit during working hours for all your complaints regarding medical applications or any issue you wish to address. Legal Representative: For those under guardianship, the guardian; for minors, the Mother-Father; in their absence, first-degree legal heirs. Signing this information form does not nullify the patient’s legal rights.
